Eligibility Criteria
Inclusion Criteria:
* Age greater than or equal to 18 years
* Hospitalization for at least a 2nd acute attack (systolic or diastolic)
* Patient living at home before hospitalization for decompensation
* Affiliated with a health insurance plan
