This trial is looking for people with type 2 diabetes and chronic kidney disease to see how the KidneyIntelX test can help manage their care. Participants will visit a primary care physician's office at Mount Sinai.
You may be able to join if
- I have been diagnosed with type 2 diabetes
- I have chronic (diabetic) kidney disease
You may not be able to join if
- I do not meet the characteristics for the KidneyIntelX test
- I have an eGFR less than 30 or 60 ml/min/1.73m2 without albuminuria
- I have end-stage renal disease (ESRD) or am on renal recovery treatments
- I am pregnant
- I am currently hospitalized
- I do not have at least 6 months of medical history before the trial
- I am taking Enbrel
