This trial is looking for adults who have high blood pressure or type 2 diabetes. Participants will receive home visits to help manage their conditions instead of going to a clinic.
You may be able to join if
- I am between 18 and 60 years old
- I speak English or Spanish
- I have high blood pressure or my blood pressure is over 140/90
- I have been diagnosed with type 2 diabetes and my A1C is over 8
- I am not pregnant
You may not be able to join if
- I am currently taking oral steroids
- I have had a solid organ transplant
- I speak a language other than English or Spanish
