This trial is looking for people aged 40 and older who have high blood pressure and currently struggle to take their blood pressure medication. Participants will receive coaching and reminder tools to help them stick to their medication and improve their health over a year.

Inclusion Criteria:
* fully insured by Blue Cross Blue Shield of Louisiana (BCBSLA)
* continuously enrolled in BCBSLA for one year
* planning to remain a member of BCBSLA for next year
* English-speaking
* telephone access
* aged ≥40 years
* diagnosis of essential hypertension (ICD-10-CM code I10)
