This trial is looking for African American adults who have survived COVID-19 and have chronic health conditions. It also seeks their carepartners who provide them with support at home.

You may be able to join if
- I am African American.
- I am at least 18 years old.
- I live in a rural or medically underserved area of South Carolina.
- I have been hospitalized or visited the ER for COVID-19 since March 2020.
- I have been diagnosed with type 2 diabetes, hypertension, cardiovascular disease, chronic kidney disease, or stroke.
- My carepartner lives nearby and is not paid for their support.
You may not be able to join if
- I am enrolled in another related clinical trial.

Inclusion Criteria:
* Male and female
* Living in a Medically Underserved Area and/or a designated rural area of South Carolina
* ≥ 18 years and above
* A previous diagnosis of one or more of the following conditions: type 2 diabetes, hypertension, cardiovascular disease, chronic kidney disease, or…
