Inclusion Criteria:
* 40 years or older
* Received treatment for COPD exacerbation in either inpatient or outpatient setting
* Referred for pulmonary rehabilitation
* Ability to understand and communicate in English
* Willingness to participate in calls with peer coach and to view storytelling vide…
