This trial is looking for older adults who are taking many medications and have several chronic diseases. Participation involves monitoring and possibly reducing the number of medications taken to improve health.
You may be able to join if
- I take 5 or more long-term medications.
- I have 3 or more chronic diseases.
- I have seen my family doctor at least once in the last 6 months.
You may not be able to join if
- I have a critically reduced life expectancy.
- I cannot visit the doctor on my own.
- I cannot take part in the informed consent process.
- I live in a nursing home.
- I have dementia or a mental or behavioral disorder.
- I am currently participating in another study about medications or chronic diseases.

Check Your EligibilityElderly GP patients are often treated with five or more medications and therefore prone to adverse drug reactions (ADR). Potentially inappropriate prescriptions (PIPs) lead to increased adverse events like falls, hospitalizations and mortality. The primary aim of this study to reduce the frequency of ADRs in multimorbid patients aged 70 years and older by reducing polypharmacy.
