Who this trial is looking for
This trial is looking for Veterans with chronic kidney disease who have trouble managing their blood pressure. Participants will work with a pharmacist to help manage their blood pressure medications and compare it to standard care.
Are You a Good Fit for This Trial?
You may be able to join if
- I am at least 18 years old
- I have chronic kidney disease stage 2, 3, or 4
- I have uncontrolled high blood pressure
- My kidney function is in a specific range
You may not be able to join if
- I have severely high blood pressure
- I take 4 or more medications for high blood pressure
- I have a significant drop in blood pressure when standing
- I have severe chronic kidney disease
- My life expectancy is less than 1 year
- I have severe liver disease
- I have severe heart failure
- I have severe cognitive decline

About This Trial
The proposed research will evaluate if patients managing their blood pressure medications under the guidance of a clinical pharmacist works better to lower blood pressure than patients monitoring their blood pressure at home and the standard care. 60 Veterans with chronic kidney disease (CKD) will be included in the study. Eligibility Criteria
Inclusion Criteria:
* Age \>= 18 years,
* CKD stage 3 or 4 (estimated GFR: 15-59 mL/min/1.73m2) or
* Stage 2 CKD (estimated GFR 60-89 mL/min/1.73m2) with urinary albumin/creatinine ratio (ACR 300 mg/g if no DM and 30 mg/g if with DM), and
