This trial is looking for people who are Medicaid patients with hypertension, diabetes, or who are pregnant. Participants will help examine how digital health care models can improve their health outcomes.
You may be able to join if
- I am a patient at a qualified health care center.
- I am insured by Medicaid or eligible for both Medicare and Medicaid.
- I have been diagnosed with hypertension, diabetes, or I am pregnant.
- I can read and understand English or translated materials.
- I am a minor who is pregnant and enrolled in the MIC Program.
You may not be able to join if
- I am not enrolled in the MIC Program.
