Who this trial is looking for
This trial is looking for people with COPD who are between 40 and 90 years old. Participants will join a 10-week program to improve their health and help address any barriers they face in getting care.
Are You a Good Fit for This Trial?
You may be able to join if
- I am between 40 and 90 years old.
- I speak English or Spanish.
- I have been diagnosed with COPD.
- I can exercise with my legs.
- I am currently prescribed COPD medication.
- I have not had a COPD flare-up in the past 6 weeks.
- I am currently receiving care within SFHN.
You may not be able to join if
- I am pregnant.
- I have dementia or a mental illness that affects my ability to participate.
- I have unstable heart disease.
- I have a serious health condition that prevents me from exercising.
- I have a contagious lung infection like tuberculosis or COVID-19.
- I have participated in pulmonary rehabilitation in the past 12 months.
- I have had a COPD flare-up in the past 6 weeks.
- I have other health issues that mean I may not live through the next year.

Eligibility Criteria
Inclusion Criteria:
* Provision of signed and dated informed consent form
* Willingness to participate in the COPD Wellness program
* 40 to 90 years old
* English or Spanish speaking
* Physician-diagnosed COPD
