This trial is looking for women who have recently given birth and had high blood pressure during pregnancy. Participants will take part in a program to help them engage in heart health care after delivery.
You may be able to join if
- I have delivered a baby and experienced high blood pressure during my pregnancy
- I can speak and read English or Spanish
- I have given my written consent to participate
You may not be able to join if
- I have been diagnosed with cardiovascular disease
- I have cognitive or vision issues that prevent communication or reading
