Older sexual and gender minority (SGM) patients are at risk for receiving inequitable end-of-life care; those with Alzheimer's disease and related dementias (ADRD) are at particularly high risk. Eligibility Criteria
Inclusion Criteria: * English speaking * 18 years and above Exclusion Criteria: --People under 18, pregnant women, prisoners, and people who are decisionally challenged will be excluded from the study.
