Who this trial is looking for

This trial is looking for adults aged 18-75 who have diabetes or prediabetes and have experienced food insecurity in the last year. Participants will receive fresh food boxes and nutrition education over six months.

You may be able to join if

  • I am between 18 and 75 years old.
  • I am a patient of Montefiore Bronx Health Collective.
  • I have experienced food insecurity in the previous year.
  • I have been diagnosed with diabetes or have an HbA1c level above 6.4%.
  • I have been diagnosed with prediabetes or have an HbA1c level between 5.7% and 6.4%.

About This Trial
Fresh Takes! is a targeted food distribution and nutrition education program offered by Montefiore Bronx Health Collective, a Federally Qualified Health Center located in the South Bronx. The goal of Fresh Takes! is to help reduce food insecurity and prevent progressions to overt diabetes in patients with prediabetes and to help patients with overt diabetes achieve better diabetic control.
