This trial is looking for Black men who have been in jail and have type 2 diabetes that isn't well controlled. Participants will work with a nurse case manager to improve their diabetes management over six months.
You may be able to join if
- I am at least 18 years old.
- I identify as Black or African American.
- I identify as male.
- I have been in a state or federal prison or county jail.
- I have been diagnosed with type 2 diabetes with HbA1c of 8% or higher.
- I can communicate in English.
You may not be able to join if
- I have significant dementia.
- I am currently in another diabetes clinical trial.
- I have active psychosis or an acute mental disorder.
- My life expectancy is less than 12 months.
- I am awaiting legal proceedings.
- I am under active surveillance by the criminal legal system.
