Inclusion Criteria:
* History of major limb amputation
* Age \> 18 years old
* Presence of phantom limb pain or negative phantom sensation(s)
Exclusion Criteria:
* Active mental illness, neurological disease, or cognitive impairment that would interfere with survey completion
