This trial is looking for adults with high blood pressure living in the Greater New Orleans area. Participants will be involved in a program led by community health workers for 18 months to help manage their condition.

- I am at least 18 years old
- I am connected to a participating church
- I have high blood pressure
- I am willing to take part in the program
- I can sign a consent form
You may not be able to join if
- I was hospitalized in the last 3 months for heart failure or heart attack
- I have current cancer treatment
- I have stage-5 chronic kidney disease
- I am pregnant or planning to become pregnant soon
- I plan to move out of the area in the next year

Eligibility Criteria
Inclusion Criteria:
1. Men or women aged ≥18 years
2. Community members associated with the participating churches (church members and their families and friends)
3. Systolic blood pressure ≥130 mmHg or diastolic blood pressure ≥80 mm Hg
