Eligibility Criteria
Inclusion Criteria:
* African-American inpatients
* Ages 18-70
* estimated glomerular filtration rate (eGFR)\<45
* Enrolled in the Hospitalist Project
Exclusion Criteria:
* Does not self-identify as African American
* Not able to consent
* Unable to speak English
