This trial is looking for hospitalized Black or African American adults with advanced chronic kidney disease. Participants will receive special education and support for 12 weeks to help manage their condition.

You may be able to join if
- I am over 18 and under 70 years old.
- I am staying at the University of Chicago hospital for more than 48 hours.
- I have been diagnosed with advanced chronic kidney disease.
- I identify as Black or African American.
You may not be able to join if
- I do not speak English.
- I am unable to communicate because of my current health.
- I cannot give consent due to my mental status.
- I am expected to stay in the hospital for less than 24 hours.
- I am currently in the ICU.

* Patients over the age of 18 and under 70
* Admitted to the University of Chicago inpatient general medicine service
* Likely hospital stay greater than 48 hours
* Diagnosis of advanced CKD (Stage 3B or above, as determine by problem list, ICD- 10 codes or eGFR\<45 during admis…
