Eligibility Criteria
Inclusion Criteria:
* Age ≥ 18 years
* Newly diagnosed HF with New York Heart Association class II-III symptoms at discharge
Exclusion Criteria
* Known prior history of HF
* Severe comorbidities that could affect protocol adherence, in the opinion of the investigator
