This trial is looking for patients with kidney failure who are on dialysis. Participants will complete a questionnaire about their health and will be followed over time to see how their heart health changes.

  • I have been diagnosed with kidney failure (CKD stage 5).
  • I am about to start or currently on dialysis.

  • I have had acute kidney injury.
  • I have severe mental illness affecting my ability to answer questions.
  • I have had a heart attack or heart surgery in the past 3 months.

  • I have serious other health issues like severe infections or tumors.
  • I am scheduled for kidney transplantation.
