- I have been diagnosed with kidney failure (CKD stage 5).
- I am about to start or currently on dialysis.

- I have had acute kidney injury.
- I have severe mental illness affecting my ability to answer questions.
- I have had a heart attack or heart surgery in the past 3 months.
- I have certain types of heart disease.

- I have serious other health issues like severe infections or tumors.
- I am scheduled for kidney transplantation.

About This Trial

Inclusion Criteria:
1. Age 18 or older with no restrictions on gender or ethnicity;
2. All enrolled patients meet the diagnostic criteria for CKD stage 5 according to the KDOQI guidelines of the National Kidney Foundation;
