This trial is looking for patients with type 2 diabetes who have a high risk of heart failure. Participants will receive an analysis of their heart health and recommendations for treatment through their healthcare records.
You may be able to join if
- I have been diagnosed with type 2 diabetes
- I have a high WATCH DM score
- I have had an echocardiogram in the last 6 months
You may not be able to join if
- I have a history of heart failure
- I am not eligible for new GLP-1RA or SGLT2i or ns-MRA
