Eligibility Criteria
Inclusion Criteria:
* Adult patients (18 years and older) admitted to five hospitals during the study period
Exclusion Criteria:
* Have less than 2 documented serum creatinine (Scr) measurements during hospitalization
* Being diagnosed with end-stage renal disease (ESRD)
