This trial is looking for African American or Black adults with advanced chronic kidney disease. Participants will receive coaching to help manage their kidney health and may help delay the need for more intensive treatments.

Are You a Good Fit for This Trial?

You may be able to join if

  • I identify as African American or Black.
  • I have had two recent tests showing my kidney function is low.
  • I have visited Emory Healthcare in the last 2 months.
  • I am willing to follow all study procedures.

You may not be able to join if

  • I am currently on dialysis.
  • I am receiving hospice care for a terminal illness.
  • I have recently been evaluated for a kidney transplant.
  • I have had a kidney transplant.
  • I am currently receiving treatment for cancer.
  • I do not speak English.
  • I am participating in another research study.
  • I am pregnant or planning to become pregnant.

Through the use of community-engaged processes, this project seeks to develop and implement clinical decision support (CDS) and a kidney health coaching (KHC) intervention.
