Who this trial is looking for

This trial is looking for adults with type 2 diabetes who are on insulin therapy and use a smartphone. Participants will use an app and messaging to help manage their diabetes over 12 months.

You may be able to join if

  • I am 18 years or older.
  • I have type 2 diabetes.
  • I am on insulin therapy.
  • I have been a Medicaid recipient.
  • I have an A1C over 7 in the last year.
  • I have an A1C over 7 at the start of the study.
  • I have owned and used a smartphone for at least 6 months.
  • I can read text messages on my smartphone.

You may not be able to join if

  • I am pregnant.
  • I have a cognitive impairment.

About This Trial

This 12-month pilot tests a nurse-led, app-based intervention to improve diabetes self-management for Medicaid patients. It includes MyChart messaging to give education, phone help on problem-solving with clinicians, and using a diabetes app to track data. The study aims to enhance self-management behaviors through health technology.

Eligibility Criteria
