Eligibility Criteria
Inclusion Criteria:
* All heart failure diagnoses
* \>18 years
Exclusion Criteria:
* Not understanding the Dutch Language.
* Allergies that make uptitration impossible.
* Admission to hospice or palliative care.
* Severe psychological disorders.
