This trial is looking for adults who are African American and/or Latinx with high blood pressure that is not well controlled. Participants will receive help managing their hypertension with a team of healthcare professionals.
You may be able to join if
- I am at least 18 years old
- I am African American and/or Latinx
- I have uncontrolled stage 2 hypertension, with blood pressure 140/90 or higher
- I follow with a Rush primary care provider
- I have a smartphone
- I have an email
You may not be able to join if
- I have received an organ transplant
- I am on dialysis
- I am already in another remote hypertension monitoring program
- I am not interested in participating
- I have already taken part in the E3 Hypertension program
- I have already taken part in the E3 Diabetes program
