This pilot study investigates the use of wearable health technology and bi-weekly digital health summaries in patients recovering from intensive care. Eligibility Criteria
Inclusion Criteria:
* Age 18-65 years at the time of ICU admission
* ICU stay of at least 48 hours
* ICU discharge within the last 2 years
* Comorbidity of diabetes mellitus and/or chronic heart failure/coronary artery disease
