This trial is looking for people with uncontrolled type 2 diabetes who have social needs that are not being met. Participants will be involved in a program designed to connect them with community resources and improve their health outcomes.

- I have been diagnosed with type 2 diabetes.
- My last HbA1c level is 9% or higher.
- I was screened for social needs at my last doctor's visit.
- I was told I have at least one unmet social need.
You may not be able to join if
- I am enrolled in Complex Care Management.
- I am receiving hospice care.
- I am deceased.
- I have type 1 diabetes.

The goal of this study is to develop, implement, and evaluate a patient-centered triage and referral model designed to improve health outcomes for individuals with uncontrolled type 2 diabetes mellitus (T2DM) and unmet health-related social needs. Eligibility Criteria
Inclusion Criteria:
* Diagnosis: Must have a diagnosis of Type 2 Diabetes Mellitus (T2DM), confirmed by a current diagnosis in the medical record or at least two billing codes in the last two years, or an HbA1c level ≥6.5% in the last two years.
