This trial is looking for people with chronic kidney disease living in rural eastern North Carolina. Participants will help test a new program designed to improve kidney health by connecting them with medical and community resources.

- I have been diagnosed with chronic kidney disease.
- I have had at least one clinic visit at Goshen Medical Center in the past year.
You may not be able to join if
- I have a limited life expectancy due to advanced cancer or liver disease.
- I am currently receiving active cancer treatment.
- I live in a skilled nursing facility.
- I have dementia or other significant cognitive impairment.

Eligibility Criteria
Inclusion Criteria:
* Diagnosis of chronic kidney disease defined as ≥1 ICD-10 CKD codes (excluding end-stage kidney disease) or CKD biomarkers (estimated glomerular filtration rate ≤ 60 ml/min, albuminuria ≥30 mg/24h)
* At least one clinic visit at Goshen Medical Center in 12 months before enrollm…
