* Critical illness:
* anticipated ICU stay \>= 7 days
* Major abdominal surgery
* elective surgery
Exclusion Criteria:
* Active malignancy
* Inherited metabolic disorder
* Known muscle disease
* Pregnancy
* Patient refusal
