Eligibility Criteria
Inclusion Criteria:
* Diagnosis of diabetes mellitus with or without diabetic retinopathy
* Scheduled visit to ZSFG clinic is specifically for diabetes mellitus or diabetic retinopathy care
* Able to speak and understand English or Spanish
