This trial is looking for adults with diabetes who receive care at certain Montefiore clinics. Participants will help test a program designed to increase the use of continuous glucose monitoring technology by their healthcare providers.

You may be able to join if
- I am 18 years old or older
- I receive primary care at a participating clinic
- I have been diagnosed with diabetes mellitus
- I am currently using insulin therapy
You may not be able to join if
- I have an existing continuous glucose monitoring prescription within the last 24 months
- I am at a clinic participating in the pilot phase of the CGM initiative
