Eligibility Criteria
Inclusion Criteria:
* Aged 18 years and older
* Hospitalized at University of Utah Medical Center (UUMC) for any medical reason, and
* Electronic Health Record (EHR) documented diagnosis of any SUD.
Exclusion Criteria:
* Currently incarcerated
