This trial is looking for adults who are about to leave inpatient rehabilitation and want to use a digital platform to help manage their health. Participants will receive personalized recommendations and provide feedback on the platform's usefulness.

You may be able to join if
- I am at least 18 years old.
- I am less than 10 days away from discharge from rehabilitation.
- I have one of the following conditions: acute coronary syndrome, COPD, minor stroke, or fragility fracture.
- I am able to provide consent.
- I have access to a device with internet.
You may not be able to join if
- I have a cognitive or psychiatric condition that affects my ability to consent or use an app.
- I have a physical disability that prevents me from using a digital device without help.
- I have serious difficulties with language that affect my understanding.

The goal of this clinical trial is to evaluate the feasibility and usability of a digital lifestyle platform designed to support patients after discharge from inpatient rehabilitation. It will also assess patient engagement and the potential of the platform to support long-term self-management and healthy lifestyle behaviors in an outpatient setting.

Inclusion Criteria:
* Adults (≥18 years)
* 10 or less days before discharge from inpatient rehabilitation at the Bern Rehab Center in Heiligenschwendi or the Insel Hospital (both belonging to the Insel Group)
