Eligibility Criteria
Inclusion Criteria:
Age ≥ 16 years MRI-confirmed high transsphincteric perianal fistula Fit for surgery (ASA I-III)
Exclusion Criteria:
* Crohn's disease
* Tuberculosis or malignancy-associated fistula
* Horseshoe fistula
* Previous definitive fistula surgery (excluding drainage procedures)
