Who this trial is looking for

This trial is looking for adults with high blood pressure and either diabetes or chronic kidney disease. Participants will use a home blood pressure monitor and get support from healthcare providers to manage their conditions better.

Are You a Good Fit for This Trial?

You may be able to join if

  • I am 18 years or older
  • I identify as non-Hispanic white, non-Hispanic Black/African American, or Hispanic
  • I have high blood pressure (140 mm Hg or higher)
  • I have been diagnosed with diabetes or chronic kidney disease
  • I receive medical care at a participating health system
  • I have a home address in Maryland or D.C.

You may not be able to join if

  • I am under 18 years old
  • I am on dialysis for end-stage renal disease
  • I have a serious medical condition that limits my life expectancy
  • I have cognitive impairment that prevents me from participating
  • I plan to leave the practice or move out of the area in 24 months
  • I do not consider this practice my primary care location anymore
  • I am unwilling to provide informed consent

About This Trial
The LINKED- HEARTS Program is a multi-level project that intervenes at the practice level by linking home blood pressure monitoring (HBPM) with a telemonitoring platform (Sphygmo).
