This trial is looking for Black and Latino adults, aged 18 to 65, who live in Harlem and have moderate risk for depression. Taking part involves being part of a community program aimed at improving mental health services and support.

You may be able to join if
- I am between 18 and 65 years old.
- I am Black or Latino.
- I live in Harlem.
- I come from a low-income housing development.
- I receive primary care services in Harlem.
- I have a PHQ-4 Total Score of 3 or higher.
You may not be able to join if
- I have severe mental illness, such as psychosis or mania.
- I have a substance abuse problem.
- I am at high risk for suicide.
