This trial is looking for adults over 18 who have recently had heart surgery. Participants will take part in home-based cardiac rehabilitation with support from their therapists through telemedicine.

- I am over 18 years old.
- I have been diagnosed with heart failure or similar heart conditions.
- I had heart surgery within the last 60 days.
- I am eligible for traditional cardiac rehabilitation.
You may not be able to join if
- I am under 18 years old.
- I have a serious heart condition (NYHA Class III or IV).
- I have experienced an acute coronary syndrome.
- I have severe heart failure.
- I have had heart surgery for structural heart disease.
- I have had a heart transplant.
- I am pregnant.

About This Trial
The aim of this study is to evaluate feasibility, efficacy, and adherence of home-based cardiac rehabilitation with the integration of telemedicine. Several components will be assessed such as quality-of-life, nutritional counseling, maximum metabolic activity (MET's), diabetic management, tobacco cessation, lipid, blood pressure, and psychosocial management. 1. Over the age of 18
2. NYHA Functional Class I, II,
3. Recent (within 60 days) status post coronary artery revascularization for atherosclerotic coronary artery disease (coronary artery bypass grafting or percutaneous coronary revascularization with stent implantation)
