This trial is looking for people who have recently given birth and may have ongoing health issues like depression, anxiety, or diabetes. Participants will receive support to help better coordinate their healthcare after pregnancy.

- I have delivered a live baby or am currently pregnant
- I have a primary care doctor listed in my medical record
- I can read and speak either English or Spanish
- I am at least 18 years old
- I have been diagnosed with depression or anxiety
- I have high blood pressure or related pregnancy issues
- I have diabetes or obesity
