This trial is looking for adults with type 2 diabetes to improve kidney screening. Participants will need to have had a visit with their primary doctor in the last two years and haven't had certain kidney tests in the past 15 months.

  • I am an adult with type 2 diabetes
  • I have seen a primary care provider in the last 24 months
  • I have not had my kidney function tested in the last 15 months

  • I have been diagnosed with chronic kidney disease
