This trial is looking for adults with type 2 diabetes and high blood sugar levels. Participants will track their glucose levels using a provided device as they take part in a program focused on nutrition.

You may be able to join if
I am at least 18 years old
I have been diagnosed with type 2 diabetes
My most recent blood test shows an HbA1c of 9% or higher
I am not using insulin
You may not be able to join if
I have type 1 diabetes
I am pregnant
I take steroids or medications that affect my glucose levels
I have a life expectancy of less than 6 months
I have stage 5 kidney disease or am at risk of needing dialysis
I have a significant disease that might affect my participation
I cannot follow study procedures

The aim of this proposal is to determine the impact of a produce prescription program in predominantly Hispanic/Latino adults with established non-insulin treated T2D and an HbA1c at or above 9%.

Life expectancy less than 6 months Diagnosis of stage 5 kidney disease or at risk of needing dialysis per Investigator discretion Any active clinically significant disease or disorder which in the investigator's opinion could interfere with participation in the study.
