This trial is looking for people who have recently had a stroke and are moving from rehabilitation to home. Participants will either learn about stroke recovery or make changes to their home environment to support their recovery.
You may be able to join if
- I am 55 years old or older.
- I have had an acute stroke diagnosed by a doctor.
- I was independent in daily activities before my stroke.
- I plan to go home after my rehabilitation.
- I can give informed consent to participate.
You may not be able to join if
- My life expectancy is less than 6 months.
- I have a cognitive impairment affecting my ability to consent.
- I have communication problems that would prevent me from completing the study.
- I live in a congregate living facility.
- I am not eligible for a therapeutic pass.
- I live outside the study area.
- The study team thinks I cannot complete the study procedures.
- I am participating in another research study that may conflict with this trial.
