This trial is looking for people who have had a stroke and high blood pressure. Participants will test two methods to help lower their blood pressure: checking it at home and learning about blood pressure in a group.

- I have had a mild to moderate stroke.
- I have a history or new diagnosis of high blood pressure.
- I can read and speak English.
- I have internet access.
- I have a primary care doctor at MGH.
You may not be able to join if
- I have had a severe stroke.
- I am going to a rehabilitation or nursing facility after discharge.
- I have dementia.
- I have moderate to severe aphasia without a caregiver.

About This Trial
The investigators want to improve care for people who have had a stroke. High blood pressure is the leading cause for having a second stroke and can lead to poor brain health. Eligibility Criteria
Inclusion Criteria:
* Incident, mild to moderate, ischemic or hemorrhagic stroke (NIHSS≤15, ICH score≤3)
* history or new diagnosis of hypertension
* ability to read and speak English
* internet access
* established primary care physician at MGH
