Who this trial is looking for
This trial is looking for people who have recently started cardiac rehabilitation. Participants will take part in fitness testing and interviews over 10 months to help understand how men and women stay active during and after their rehab.

- I plan to move out of the area during the study
- I have a medical procedure planned that would limit my activity

- I have had a heart transplant
- I use a left ventricular assist device
- I am pregnant or planning to become pregnant during the study
- I have another illness that would affect my participation

About This Trial
The SPARC study will measure physical activity and functional fitness during cardiac rehabilitation, right after completion of cardiac rehabilitation, and 6 months later. This will help investigators learn if men and women maintain physical activity levels and functional fitness differently during and after they complete cardiac rehabilitation. Inclusion Criteria:
* Age \> 18 years
* Enrolled in cardiac rehabilitation within the last month
* Own a smartphone or tablet for mobile application download

* Planned relocation during the study period out of the local area
