You may be able to join if
- I have been diagnosed with type 2 diabetes
- I have an HbA1c between 6.5% and 10%
- My eGFR is between 30 and 90 mL/min/1.73 m²
- I have an urine albumin to creatinine ratio between 500 and 5000 mg/g
You may not be able to join if
- I have a history of non-diabetic kidney disease
- I have needed treatment with strong immune-suppressing drugs
- I am medically unstable according to my doctor
- I was hospitalized for more than 24 hours in the last 30 days

Key Inclusion Criteria:
1. Medical history of type 2 diabetes and receiving medical therapy or lifestyle interventions for glucose management
2. Hemoglobin A1C (HbA1c) of 6.5 to 10% at screening
