This trial is looking for pregnant women aged 18-45 who have chronic hypertension. Participants will have telehealth visits and phone or video sessions with a health coach until their baby is born.

- I have been diagnosed with chronic hypertension.
- I am pregnant.
- I am on medication for high blood pressure.

- I have a serious ongoing medical or mental health condition.
- I have a heart condition like angina or heart disease.

- I plan to have my baby outside Atrium Health Wake Forest Baptist.
- I have had a stroke before.
- I have severe high blood pressure.

The purpose of this research study is to compare the management of chronic hypertension during pregnancy using telehealth visits with blood pressure telemonitoring and health coaching as compared to standard in person clinic visits. Inclusion Criteria:
* prior diagnosis of Chronic hypertension (cHTN)
* Pregnant women
* Known: documented diagnosis or on antihypertensive medication prior to pregnancy
