This trial is looking for adults with high blood pressure who visit the emergency department. Participants will answer questions about their health and receive help connecting to local services for their needs.
Are You a Good Fit for This Trial?
You may be able to join if
- I am 18 years or older
- I live in one of the five boroughs of NYC
- I speak English or Spanish
- I have been diagnosed with hypertension
- My blood pressure is not controlled
- I had a systolic blood pressure of 140 mmHg or higher or diastolic blood pressure of 90 mmHg or higher during my ED visit
You may not be able to join if
- I am unable to consent
- I am pregnant
- I have a history of end stage renal disease
- I have a history of heart failure
- I have a BMI greater than 50

About This Trial
This single-center, single-arm quasi-experimental study will assess the feasibility and acceptability of a community health worker (CHW)-led intervention to address social determinants of health (SDOH) among emergency department patients with hypertension. Inclusion Criteria:
* 18 years and older who reside in the 5 boroughs of NYC,
* whom speak English or Spanish,
* have a known diagnosis of hypertension, and
