This trial is looking for children aged 5 to 18 with asthma who are receiving care at a specific clinic. Participants will help test new ways to improve asthma treatment using a method called SMART.

- I am between 5 and 18 years old.
- I have been to the participating clinic for a visit.
- I have been prescribed an inhaled corticosteroid or a combination inhaler in the past year.
- I have uncontrolled asthma based on specific assessments.

- I have transferred clinics or left the CHOP Pediatric Care Network.

Clinic Inclusion Criteria:
* The clinic is a pediatric primary care clinic that is part of the Children's Hospital of Philadelphia (CHOP) Pediatric Research Consortium (PeRC).
* The clinic agrees to participate in SMART \& SIMPLE study.
