Parkinson's clinical trials in Michigan: what to expect
Parkinson's trials fall into three groups. Disease-modifying studies test drugs meant to slow progression, including treatments aimed at alpha-synuclein, repurposed diabetes drugs and therapies for people with LRRK2 or GBA gene variants. Symptom studies test new ways to smooth out "off" periods and dyskinesia, and procedures such as focused ultrasound and deep brain stimulation. And a large number of studies involve no medicine: exercise, speech and balance programs, wearables, and long-term observational studies that track the disease.
In Michigan, recruiting parkinson's studies are concentrated in Farmington Hills, Ann Arbor and East Lansing. 15 of the 26 are run by a drug or device company, which usually means a larger study with travel reimbursement; the rest are academic or hospital-led.
Who Parkinson's trials usually look for
Disease-modifying trials usually enroll people diagnosed in the last two to five years, sometimes before any Parkinson's medicine has started. Symptom trials want people on levodopa who have a stated number of hours of "off" time a day. Most set ages 30 to 80, exclude dementia and atypical parkinsonism, and may require a dopamine transporter scan to confirm the diagnosis. Genetic trials test for the variant at no cost. What taking part involves
Screening includes a movement exam, sometimes performed both before and after your morning dose, blood work and a scan. Drug trials run twelve weeks to two years with monthly visits, symptom diaries and, in some, a spinal fluid test. Placebo arms are common and medicines you already take generally continue. Exercise and rehabilitation trials involve sessions two or three times a week for several months. Everything study-related is free and travel is usually reimbursed; a care partner is welcome and sometimes required.
